Peter Katris, D.D.S. (Elgin Dental Associates and Carol Stream Dental Associates)

Dr. Peter Katris is the founder of Carol Stream Dental Associates and Elgin Dental Associates, multi-specialty practices established in 1992 and 1994 respectively. Dr. Katris received his dental degree from Northwestern University Dental School in 1991. With a mindset of comprehensive dentistry, Dr. Katris believes in working closely with specialists in different phases of dentistry to achieve excellent results.

Dr. Katris enjoys all phases of restorative and cosmetic dentistry and has a passion for beautifying smiles. He has accumulated several hundred hours of continuing education in all aspects of dentistry including implants, porcelain veneers, onlays, and highly aesthetic composite veneers and fillings. Dr. Katris stays on the cutting edge of dentistry by continuing his active participation in advanced training classes throughout the year; including at the University of Minnesota certificate program in aesthetic dentistry. Dr. Katris is an active member of the American Dental Association, the Chicago Dental Society and the Lake Cook Study Club. Dr. Katris enjoys time with his family, cooking and excercising.

Timothy Hang, D.D.S. (Elgin Dental Associates and Carol Stream Dental Associates)

Dr. Timothy Hang joined Elgin Dental Associates and Carol Stream Dental Associates in December of 2001. He graduated from the University of Illinois College of Dentistry in 1997. He then completed a residency in Hospital Dentistry in 1998, focusing on treating medically comprised dental patients. Dr. Hang practices all phases of general dentistry.

Amy Johnson Kim, D.D.S. ( Carol Stream Dental Associates)

At a young age, Dr. Amy Johnson Kim was inspired by her family dentist back home in Green Bay and knew she wanted to pursue dentistry as a career.  On her path, she first attended the University of Wisconsin, Madison obtaining a Bachelor of Science in Psychology.  She went on to receive her dental degree in 2003 from Marquette University, School of Dentistry in Milwaukee.  Following graduation, Dr. Kim chose to advance her clinical knowledge and skills in post-doctorate programs at VA Hospitals in Minneapolis and Los Angeles.  It was there she had the opportunity to teach and mentor dental students, gain invaluable experience working with specialists in all areas of dentistry, and most importantly, learn how to safely and competently care for patients with complex medical and dental conditions.  Over the years, Dr. Kim has treated patients in private practice and kept abreast on the latest techniques dentistry has to offer in order to provide her patients with the best possible care.  With an artistic eye, she enjoys cosmetic dentistry, but believes first and foremost in achieving optimal oral health.  She focuses on getting to know each and every patient, and feels that understanding their fears and concerns and ensuring they are comfortable are the keys to successful dentistry.  Dr. Kim’s goal is to instill open communication, calmness, and confidence in her patients and she strives for excellence and satisfaction in everything she does.  

In her free time, Dr. Kim enjoys spending time with her family, photography, cooking, traveling and is always working on some creative project. 

Johnny I Kuttab, D.D.S.  Diplomat of the American Board of Pediatric Dentistry       (Elgin Dental Associates and Carol Stream Dental Associates)

Dr. Johnny Kuttab grew up in Hoffman Estates and received his Bachelor degree in Biological and Dental Sciences from UIC in 2004.  He continued on and received his Doctorate in Dental Surgery in 2008, and completed his post doctorate education in atric Dentistry at Children’s Memorial Hospital/Northwestern University in 2010. He is Board Certified and attending at Lurie Children's Hospital of Chicago.  He is newly married to a Pharmacist from Mississippi, Vera. 

Dr. Kuttab's practice philosophy is simple: "I want to provide quality pediatric preventative and operative dental care to all children, while always focusing on treating the whole child in a positive, rewarding, and psychologically advantageous setting.  I practice and hold myself to the highest standards of care".  

Jay Patel, D.D.S., Orthodontist (Elgin Dental Associates)

Dr. Patel was born in Morogoro, Tanzania. He spent his pre-teen and early teen years in Ndola, Zambia, and then spent his high school years in London, England. He then came to the United States in 1979, and finished his undergraduate degrees in Chemistry and Physiology at Southern Illinois University in 1981 and 1982.

Dr. Patel completed his Doctorate degree in Dental Surgery from Northwestern University in Chicago, Illinois in 1986 and completed a residency in Orthodontics at Indiana University in 1988. He is a member of the Chicago Dental Society, American Dental Association, Midwestern Society of Orthodontics, and America Association of Orthodontics. Throughout the year he attend continuing education courses in orthodontics to keep abreast of the latest developments. Dr. Patel has been a part of Elgin Dental Associates since 1997.

Michael Slavin D.D.S.,  Periodontist                                                                                   (Elgin Dental Associates and Carol Stream Dental Associates)

Dr. Michael Slavin graduated from the University of Illinois-College of Dentistry in 1995. He then received a degree in Periodontics from Northwestern University in 1998 and a degree in Dental Anesthesiology in 2000.

He is a member of the Chicago Dental Society, the American Dental Association, the American Academy of Periodontists. Dr. Slavin has been part of both Carol Stream Dental Associates and Elgin Dental Associates for over seven years.
 

Rudolfo Valente, D.D.S., M.S., Orthodontist (Carol Stream Dental Associates)

Dr. Rudy Valente has been in orthodontic practice since 2001.  He earned a Bachelor of Science degree in Biology from the University of Illinois at Urbana/Champaign.  Dr. Valente earned his dental degree and received his certificate in orthodontics from the University of Illinois at Chicago: College of Dentistry.  In conjunction with his residency Dr. Valente also received a Master of Science degree in Oral Sciences for his research with orthodontic bonding agents which has been published in the American Journal of Orthodontics and Dentofacial Orthopedics.

In addition to taking excellent care of children and adolescents Dr. Valente has also taken courses at the Pankey Institute and is well versed in adult interdisciplinary dental rehabilitation.

Dr. Valente is a Diplomate of the American Board of Orthodontics.  He is also an active member of the American Dental Association, the American Association of Orthodontists, the Chicago Dental Society, the Illinois State Dental Society, and the Illinois Society of Orthodontists.
